This position is involved in coordinating the Case Management Program, which consists of planning, arranging for and coordinating appropriate home and community-based services which should be provided to clients in a timely and cost-effective manner.  The person assigned to this position maintains considerable contact with service providers, clients, and works with a Registered Nurse case manager in the team approach to screen referred clients and determine eligibility.
